Affirmative Action & Self-Identification forms

A comprehensive description of the affirmative action process at York University as outlined in the collective agreement can be found by clicking AA process. The following two forms are used as part of the AA process:

  1. A copy of the candidate SELF-IDENTIFICATION FORM can be obtained by clicking Self-ID form.
  2. A copy of the candidate WORK STATUS DECLARATION FORM (formerly called the Citizenship form) can be obtained by clicking Work-Status form, or for Contractually Limited Appointments by clicking Work-Status (CLA) form.

Affirmative Action Statement for Advertisements

All advertisements shall include the following statement :
